Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team, who will play a pivotal role in supporting the SHS/EM/MA Fulfillment. This position requires a compact nursing license and involves leading a multidisciplinary team to deliver comprehensive, person-centered care management for a diverse population with various health and social needs. The Case Manager serves as the primary point of contact for members, caregivers, and providers, utilizing multiple communication channels including phone calls, emails, and online messaging platforms.
Key Responsibilities
• Lead the coordination of a multidisciplinary team to provide holistic care via telephonic and digital methods.
• Utilize the case management process to assess, develop, implement, monitor, and evaluate care plans aimed at optimizing member health across the care continuum.
• Assess members’ health, psychosocial needs, cultural preferences, and support systems.
• Collaborate with members and caregivers to create individualized care plans, addressing barriers and identifying gaps in healthcare.
• Arrange necessary resources to fulfill identified needs, including community resources and healthcare services.
• Coordinate care delivery and support among members, providers, and community agencies.
• Advocate for members and promote self-advocacy.
• Deliver education related to health literacy, self-management skills, and medication adherence.
• Monitor and evaluate the effectiveness of care plans, adjusting as necessary to ensure progress toward goals.
• Prepare members and caregivers for discharge or transfer, ensuring continuity of care.
• Document interactions accurately to support member management.
• Adhere to professional standards and engage in continuous professional development.

Qualifications
• Nursing Diploma or Associate’s degree in nursing required.
• Bachelor’s degree in nursing preferred.
• Minimum of 3 years clinical nursing experience in diverse settings required.
• At least 1 year of case management experience in a managed care setting preferred.
• Experience managing patient care through telephonic and digital channels is advantageous.
• Current, active, and unrestricted Michigan Registered Nurse license required.
• Certification in Case Management (CCM) required or to be obtained within 18 months of hire.
• Certification in Chronic Care Professional (CCP) is preferred.
